| Connected Kids Goes to School |
Title: CONNECTED KIDS GOES TO SCHOOL
Phyllis Agran, MD, MPH1,2, Jyoti Trueman, MPH1, Amy Dale, MPH1, Bethi Luu, BA1, Jamie Mc Donald, MPH1, Debra Hill3, Joey Van Camp, RN CPNP3, Carolyn Moser, MD1,2 and Annelee Gallegos, BS1. 1Injury and Violence Prevention Program, California Chapter 4, American Academy of Pediatrics, Garden Grove, CA, United States, 92840; 2Emergency Medicine and Pediatrics, University of California Irvine, Orange, CA, United States, 92868 and 3Westminster School District, Westminster, CA, United States.
Background Connected Kids (CK), an AAP violence prevention program for anticipatory guidance, was adapted for use in school. The pilot program, Connected Kids Goes to School, reaches children 11 - 14 years, who visit the pediatrician less often than younger children. Collaboration was established with the multi-ethnic, socioeconomically diverse Westminster School District to expose children to CK messages.
Setting/population School professionals, community organizations and law enforcement participated at the School District.
Program activities A focus group with 13 multidisciplinary school nurses, counselors, psychologists, teachers and after school program educators was conducted to introduce and determine how to adapt CK within the school environment. Data were used to plan a workshop; adapt the Clinical Guide and determine priority CK topics. Follow-up includes an electronic survey to determine if/how CK is used, follow-up meeting, and school-based additions to the Clinical Guide.
Results Priority CK topics were Bullying, Talking to Your Teen and Staying Cool. CK brochures in Spanish and Vietnamese were requested and adapted with focus group testing. 38 multidisciplinary participants attended the workshop. They will incorporate CK into parent/teacher/nurse conferences, after school programs, parent nights, team sports, newsletters and peer mediator training. Brochures will be displayed in school and community settings. Evaluation outcomes included requests for additional workshops and collaboration with CA4, AAP, and additional CK brochures. School District leadership will conduct additional focus groups, use brochures and involve community partners in CK. Requests for community trainings were also generated.
Conclusions and Implications The CK Goes to School pilot was successful in capturing the enthusiasm of school professionals who identified the need for CK brochures and recognized the value of consistent messages and integrating CK in the school environment. The School District will continue implementation and use of materials. Schools have the advantage of dealing with children/families on a daily basis and are excellent partners for AAP chapters. |

Pediatric Research in Office Settings (PROS):
Providing the Evidence For New Guidelines on Very Young Febrile Infants
Harry Pellman, MD, FAAP
How does participation in Pediatric Research in Office Settings (PROS http://www.aap.org/pros), the AAP's practice-based research network, enhance pediatric practice and improve care for children? Here's one way.
Do you remember all those young infants you saw during your training who presented to the emergency department who presented with fever? Nearly all of those children were admitted, worked up for sepsis, and placed on antibiotics pending culture results. That management strategy was based on guidelines which, in turn, had been based on evidence from pediatric emergency departments.1 Those of you have been in general pediatrics practice for a few years know by now that this management strategy is not followed uniformly by practicing pediatricians, who see a different spectrum of patients. The PROS Febrile Infant Study demonstrated that practicing pediatricians using management strategies less aggressive than those suggested in the guidelines can achieve good outcomes in very young febrile infants.2
Thinking that a new practice guideline might be created that took into account these new data, PROS tapped Febrile Infant Study principal investigator Bob Pantell to spearhead an effort to get the AAP behind creation of a new guideline. He did so, and the AAP in turn nominated Febrile Infants < 3 Months Old to the Agency for Healthcare Research and Quality (AHRQ) for new guideline development. Recently, we learned that one of AHRQ's Evidence-Based Practice Centers is beginning work on a new guideline. This new guideline ultimately will benefit both practicing pediatricians, who no longer will risk being "hung out to dry" for less aggressive management of young febrile infants that is supported by existing evidence (developed by PROS practitioners), and selected young infants and their families, who will safely avoid unnecessary testing, treatment, and hospital admission.
Want to help develop evidence that leads to a change in pediatric practice? Consider joining PROS. Any pediatric practice or clinic with at least one AAP member is eligible to join PROS.For more information, please contact me at hpellman@earthlink.net, or contact PROS Central at 800/433-9016, extension 7623.
